Structured day support — recreation, learning, social connection — anchored to the person's strengths and the goals in their plan.
For individuals with high medical or behavioural complexity. LPN oversight, clear escalation pathways, robust documentation.
Bathing, transfers, toileting, mealtime, medication. Done with skill, dignity, and the right pace for the day.
Positive Behaviour Support (PBS) consistent with your existing team plan. We collaborate with OT, SLP, psychology and case management.
Planned, emergency, overnight or extended respite — so caregivers can rest without rearranging the household.
If you are a PDD or FSCD case manager, social worker, or healthcare colleague placing a client with us — we make handover easy. Plans and progress notes are written to be read by clinicians, families, and the person at the centre.
